PART 2 — THE SHADOW ON THE SCREEN Dr. Ortiz stared at the frozen ultrasound image for so long that I finally stopped looking at the monitor and started looking at her. Doctors learn how to hide fear. Mothers learn how to recognize it anyway. “What is that?” I asked.
Dr. Ortiz didn’t answer immediately. The ultrasound technician adjusted the brightness on the monitor, then enlarged one section of the image. There was no tiny heartbeat. No curled little body. No unmistakable shape of a baby. Instead, there was a dark, irregular shadow low on the right side of Sophie’s abdomen. The technician measured it once. Then again.
Then a third time.
Sophie squeezed my fingers.
“Mom?”
“I’m right here.”
I looked at Dr. Ortiz.
She finally turned toward us.
“No, Sophie.”
My lungs released a breath so violently that it almost sounded like a sob.
“No baby?”
“No.”
Sophie’s whole body sagged against the pillow.
For one beautiful second, relief flooded through me.
I actually laughed.
Then I saw that Dr. Ortiz wasn’t smiling.
The relief disappeared instantly.
“What’s wrong?”
She pulled the stool closer.
“The ultrasound does not show a pregnancy.”
I swallowed.
“Then why are her tests positive?”
“That is what we need to understand.”
She pointed toward the shadow on the screen.
“We found something near Sophie’s right ovary.”
My mouth went dry.
“Something?”
“A mass.”
The word seemed too large for the room.
Sophie looked between us.
“What’s a mass?”
Dr.
Ortiz spoke carefully. “It means there is tissue growing where we didn’t expect to find it. Sometimes these things are harmless. Sometimes they need treatment. We need more tests before we know what this is.” I stared at the screen again. “How big?” The technician answered quietly. “Just under seven centimeters.” Seven centimeters. My ten-year-old daughter weighed sixty-eight pounds. Seven centimeters sounded enormous. “Could that be causing the positive pregnancy test?” Dr. Ortiz nodded slowly. “Possibly.”
I must have looked completely lost because she continued.
“Pregnancy tests detect a hormone called hCG. Pregnancy is the most common reason that hormone appears, but it is not the only reason. Certain rare tumors can also produce it.”
Tumor.
That word was worse than pregnancy.
Much worse.
I covered my mouth.
Sophie heard it too.
“What’s a tumor?”
I immediately said, “Honey—”
But Dr. Ortiz shook her head gently.
She wasn’t going to lie to her.
And somewhere deep inside me, I was grateful for that.
“A tumor is an abnormal growth,” she said. “Some tumors are dangerous. Some aren’t. Right now, we do not know what yours is.”
Sophie stared down at her stuffed bunny.
She had brought it from the car because she said hospitals smelled lonely.
Now she rubbed one ear between her thumb and forefinger.
“Am I going to die?”
My heart broke.
“No.”
The word came from me so fast and so sharply that everyone in the room looked at me. I leaned over her. “No, Sophie. You listen to me. We don’t know what this is yet, and nobody is going to decide the worst thing before we have answers.” Her chin trembled. “Promise?” I held her face between my hands. “I promise that I’m staying beside you for every single second.” It wasn’t the promise she had asked for. It was the only one I knew I could keep. Outside the room, voices suddenly rose again. Eric. “I want him away from my daughter!” Daniel answered, but I couldn’t make out the words. Then a security officer spoke. The argument became louder. Sophie flinched. “Mom, please make Dad stop yelling at Daniel.” I stood. Anger replaced fear so quickly it startled even me. I walked into the hallway and pulled the door shut behind me. Eric was standing six feet from Daniel. Two security officers had positioned themselves between them. The social worker, a woman named Ms. Bennett, was trying to calm everybody down. Eric pointed at Daniel the second he saw me. “You need to tell them.” “Tell them what?” “That he lives with her. That he’s alone with her.” Daniel looked like someone had drained every drop of blood from his face. I stepped between them. “There is no pregnancy.” Eric blinked. “What?” “There’s no baby.” For the first time since arriving at the hospital, he became silent. “The ultrasound found a mass near Sophie’s ovary.” His expression changed. Not relief. Not exactly. Confusion. “A mass?” “The doctors think it may be producing the hormone that caused the positive test.” Daniel closed his eyes. His shoulders dropped. Eric stared at him. I could see the exact second shame started crawling across his face. But then he looked at me. “So they’re sure?” “They’re sure there is no pregnancy.” “That means—” “Yes.” I didn’t raise my voice. I didn’t need to. “You accused my husband of abusing Sophie in the middle of a hospital hallway before anyone had the facts.” Eric’s jaw tightened. “What was I supposed to think?” “You were supposed to think about your daughter before your anger.” “I was protecting her.” “No. You were looking for somebody to blame.” His eyes flicked toward Daniel. There was history between them. Years of it. Eric had hated Daniel from the moment I remarried. Not because Daniel had ever treated Sophie badly. The opposite was true. Daniel attended school plays. Daniel packed lunches. Daniel sat through parent-teacher conferences when Eric forgot them. Daniel knew Sophie hated crusts on sandwiches and loved astronomy and always needed the hallway light left on at night. Eric loved Sophie. I had never doubted that. But loving your child and showing up for your child were not always the same thing. And Eric knew it. Maybe that was why he hated Daniel so much. Daniel represented every moment Eric had missed. Ms. Bennett stepped closer. “Mrs. Morgan, even with the new medical information, we still need to complete our assessment.” “I understand.” Eric looked offended. “Why?” “Because a positive pregnancy test in a ten-year-old automatically triggered safeguarding procedures. We document what happened, including the medical findings.” Daniel nodded. “Whatever you need.” Eric finally looked at him. Daniel met his eyes. For several seconds, neither man spoke. Then Eric said, “I shouldn’t have said that.” Daniel’s face remained expressionless. “No.” “I panicked.” “I know.” “I saw Sophie crying and—” Daniel interrupted him. “You pointed at me and told security to arrest me.” Eric looked down. Daniel stepped closer. One security officer immediately raised his hand, but Daniel stopped. “I have helped raise that little girl since she was four years old.” His voice cracked on the final word. “I have cleaned vomit off her pillow. I have sat on the bathroom floor when she had the flu. I taught her how to ride a bicycle. I stayed awake all night when she had pneumonia because her mother was exhausted.” Eric said nothing. Daniel continued. “And you needed thirty seconds to turn me into a monster.” I had never seen Daniel cry. Not when his father died. Not when we lost our first pregnancy. Not even when our house flooded and we spent six months fighting the insurance company. But there were tears standing in his eyes now. Eric noticed them. So did I. “I’m sorry,” Eric whispered. Daniel nodded once. But he didn’t say he accepted the apology. And I understood why. Some accusations disappear when the truth comes out. Others leave a stain. Before anybody could say more, the examination room door opened. Dr. Ortiz stepped into the hallway. “We’re taking Sophie for a CT scan.” My attention snapped back to her. “Why?” “We need to see the mass more clearly.” “Right now?” “Yes.” Something in her tone terrified me. “What aren’t you telling me?” She hesitated. That hesitation told me everything. “There are characteristics on the ultrasound that concern me.” Eric stepped forward. “What characteristics?” Dr. Ortiz kept her eyes on me. “The borders are irregular. And there appears to be increased blood flow around the mass.” My knees weakened. “Cancer?” “We cannot diagnose cancer from an ultrasound.” “But you’re worried about it.” “I’m concerned enough that I’ve already contacted pediatric oncology.” The hallway tilted. Daniel caught my elbow. “No,” I whispered. Dr. Ortiz touched my arm. “This is not a diagnosis.” “She’s ten.” “I know.” “She came here because she had headaches.” “And those symptoms may be connected.” “How?” “We need the scan.” Behind us, Sophie called from the room. “Mom?” Every adult in the hallway went silent. I wiped my face before turning around. Then I walked back inside with a smile I absolutely did not feel. “I’m here.” “Where are they taking me?” “To get better pictures.” “With the big donut machine?” Despite everything, I smiled. “The big donut machine.” She frowned. “Last time you told me it was a spaceship.” “You said that was childish.” “I was eight.” “Ancient history.” A tiny smile appeared. Daniel stood at the doorway because the social worker still hadn’t officially cleared him to enter. Sophie saw him. “Can Daniel come?” Ms. Bennett looked at Dr. Ortiz. Dr. Ortiz looked at me. I looked at Sophie. “Yes.” Daniel entered. Sophie immediately reached toward him. He crossed the room and took her hand. Eric stood outside watching through the glass. When Sophie came back, she announced that the machine was “less scary than the dentist but more scary than the school cafeteria.” Daniel laughed. I tried. A woman in dark blue scrubs walked beside her. She had silver-streaked hair and the calmest eyes I had ever seen. “Mrs. Morgan, this is Dr. Priya Shah. She’s one of our pediatric oncologists.” The room changed the moment I heard the word. Oncologist. Sophie didn’t know what it meant. I did. Daniel did too. His hand tightened around mine. Dr. Shah sat across from us. “I’ve reviewed Sophie’s imaging.” I could hear my heartbeat. “There is a tumor involving her right ovary.” Tumor. Not mass anymore. Tumor. “How certain are you?” “Very certain that it is a tumor. We do not yet know exactly what type.” “Is it cancer?” “We need tissue analysis before we can answer that definitively.” Sophie watched my face. I forced myself to stay calm. Dr. Shah continued. “However, the combination of her age, the elevated hCG level, and the appearance of the mass suggests a type of germ cell tumor.” I had no idea what that meant. Daniel asked, “Is it treatable?” “Yes.” One word. One beautiful word. I grabbed onto it. Dr. Shah continued. “Many pediatric ovarian germ cell tumors respond very well to treatment, particularly when identified early.” “When can you remove it?” “That’s what we need to discuss.” She opened Sophie’s scan on the computer. The tumor appeared like a pale gray knot inside her tiny body. I hated it instantly. It looked like an intruder. Something that had entered my child without permission. Something that had spent weeks sending signals through her blood, creating headaches and nausea and confusing her body into behaving years older than it should. Dr. Shah pointed toward the image. “The tumor appears localized, which is encouraging.” I squeezed Daniel’s hand. “But?” Doctors always had a but. “There is something else.” Of course there was. “What?” Dr. Shah changed the image. This scan showed more of Sophie’s abdomen. She pointed toward several small areas higher up. “We see enlarged lymph nodes.” My heart sank. “What does that mean?” “It could mean inflammation.” “Or?” “It could mean the tumor has spread.” The word spread shattered something inside me. Sophie looked up. “Mom, what does spread mean?” I froze. Dr. Shah answered before I could. “It means we have to investigate some other places in your body to make sure everything is healthy.” Sophie considered this. Then she nodded. “Okay.” Just okay. Children can accept terrifying things with a simplicity adults spend their whole lives trying to recover. She picked up her stuffed bunny. “When can I go home?” Nobody answered. Her face changed. “I can go home tonight, right?” Dr. Shah leaned closer. “I think we should keep you here.” “But tomorrow is science fair.” I closed my eyes. I had forgotten. For three weeks, Sophie had been building a model of Saturn using foam balls and fishing wire. The rings were still drying on our dining room table. “I’m supposed to explain why Saturn floats in water.” Daniel smiled weakly. “You’ve explained that to me approximately four hundred times.” “Because you keep forgetting.” “I’m old.” “You’re thirty-eight.” “Practically fossilized.” She giggled. remained near the doorway. He looked smaller now. The rage was gone. He walked toward Sophie. “Hey, peanut.” She looked at him. “Why did you yell at Daniel?” Eric stopped. There are moments when adults decide whether to give children convenient lies or uncomfortable truths. Eric chose truth. “Because I got scared.” “You said he hurt me.” “I was wrong.” “You told them to arrest him.” “I know.” Sophie looked at Daniel. Then back at Eric. “You need to apologize.” “I did.” “No.” Eric blinked. “I mean really apologize.” Daniel almost smiled. Sophie sat straighter. “Ms. Carter says when you hurt somebody, you have to say what you did wrong. You can’t just say sorry.” Eric looked at his ten-year-old daughter. Then at Daniel. He took a breath. “I accused you of something horrible without knowing what happened.” Daniel listened. “I said it loudly where Sophie could hear me, and where strangers could hear me. That was wrong.” Daniel nodded. “And?” Sophie prompted. Eric almost laughed. “And I am sorry.” Sophie looked at Daniel. “Okay?” Daniel rubbed the back of his neck. “Okay.” Apparently the ten-year-old had negotiated peace between two grown men in under ninety seconds. Then Sophie leaned toward Eric. “You have to apologize to Mom too.” Eric sighed. “I walked into that one.” For a few minutes, the hospital room almost felt normal. Then Sophie suddenly went quiet. Her smile disappeared. She pressed a hand against her lower stomach. “Mom.” “What?” “It hurts.” I stood. “How bad?” She curled slightly. “Really bad.” The monitor beside her started beeping faster. Daniel hit the nurse call button. “Sophie?” She gasped. I had never heard that sound from my daughter before. Not during broken bones. Not during stitches. Not during the time she fell off the monkey bars. This was different. Primitive. Terrified. “Mom!” I climbed beside her. “I’m here!” The door opened. A nurse rushed inside. Then another. Dr. Ortiz appeared seconds later. Sophie was crying now. “My stomach! My stomach!” Dr. Ortiz pressed gently against her abdomen. Sophie screamed. “Get Dr. Shah.” The nurse ran. “What’s happening?” I demanded. “We need another ultrasound.” “You just did one.” “The tumor may have twisted the ovary.” “What does that mean?” “Ovarian torsion. It can cut off the blood supply.” Daniel went pale. Dr. Shah rushed inside. She took one look at Sophie and started giving orders. “Call surgery.” My head snapped toward her. “Surgery?” “If this is torsion, we may not be able to wait.” Suddenly there were six people in the room. Someone disconnected Sophie from one monitor and attached her to another. A nurse pulled the brakes off the bed. Another handed me a consent form. Words blurred across the page. Emergency laparoscopic exploration. Possible removal of right ovary. Possible tumor resection. Possible biopsy. I stared at the signature line. “She’s ten.” I didn’t even realize I had said it aloud. Dr. Shah put a hand on my shoulder. “I know.” “Will she be able to have children someday?” It felt insane to ask that question while my daughter screamed in pain. But motherhood turns your mind into a thousand futures at once. Birthdays. Graduation. Marriage. Children she might want. Children she might not. A life that should have been decades away. I grabbed it. Then the bed moved farther. Our fingers slipped apart. “Mom!” “I’m right here!” The doors closed. And she was gone. I stood staring at the doors long after they shut. Daniel wrapped his arms around me. I turned and buried my face against his chest. For the first time all day, I completely fell apart. “I should have brought her sooner.” “No.” “I thought they were headaches.” “You took her to the pediatrician.” “I should have known.” “How could you possibly have known?” “I’m her mother.” “That doesn’t make you psychic.” Eric stood several feet away. He looked like he wanted to come closer but didn’t know whether he had the right. Finally he said, “She’s going to be okay.” I looked at him. “You don’t know that.” He swallowed. “No.” Then he sat beside us. For the next two hours, nobody fought. Nobody blamed anyone. Nobody mentioned custody schedules or missed weekends or child support or old arguments. Three adults simply sat in plastic chairs waiting for one little girl to come back through a pair of doors. At 6:42 p.m., Dr. Shah appeared. I jumped so quickly my knees struck the table. “How is she?” “She’s stable.” I almost collapsed. Daniel caught me. “Did you remove the tumor?” “Yes.” “And?” “The ovary had twisted twice. The blood supply was severely compromised.” “Did you have to remove it?” “Yes.” I closed my eyes. “But the left ovary appears completely healthy.” I nodded. “Okay.” It wasn’t okay. But Sophie was alive. So okay would have to be enough. Dr. Shah continued. “We removed the tumor intact and sent it to pathology.” “When will we know if it’s cancer?” “We should have preliminary results soon.” “How soon?” “Possibly tomorrow.” Eric asked, “What about those lymph nodes?” “We did not remove them tonight. The priority was the torsion and the ovarian tumor.” Daniel studied Dr. Shah’s face. “You said she’s stable.” “Yes.” “But you still look worried.” Dr. Shah didn’t respond. My stomach dropped. “What happened?” “There was something unexpected during surgery.” Of course there was. There was always something else. “What?” Dr. Shah pulled out her phone and opened a photograph from the procedure. I didn’t understand what I was looking at. Daniel leaned closer. Neither did he. Dr. Shah enlarged the image. “When we examined Sophie’s abdominal cavity, we found a very small second growth.” “Another tumor?” “Possibly.” “Where?” She pointed. “It was attached near her appendix.” My chest tightened. “Did you remove it?” “Yes.” “What does that mean?” “We don’t know yet.” That sentence had become the soundtrack of my entire day. Dr. Shah put the phone away. “But there’s something unusual about it.” “What?” “The second growth looked completely different from the ovarian tumor.” Eric frowned. “So?” “So it may be unrelated.” I stared at her. “You’re telling me my daughter could have two different tumors?” “I’m telling you we need pathology.” Daniel asked the question I couldn’t. “How likely is that?” “Rare.” “How rare?” “Rare enough that I don’t want to speculate.” That frightened me more than anything else she had said. Dr. Shah started walking away. Then she stopped. “There’s one more thing.” I nearly laughed. Not because anything was funny. Because apparently nightmare number three had arrived before nightmare number two had even finished introducing itself. “What now?” “The laboratory repeated Sophie’s tumor markers while she was in surgery.” “And?” “Her hCG level increased significantly from the measurement taken this morning.” “But you removed the tumor.” “The sample was taken before removal.” “So that makes sense?” “Possibly.” There was that word again. Possibly. “What other marker did you test?” Dr. Shah hesitated. “AFP. Alpha-fetoprotein.” “And?” “It is elevated too.” “What does that mean?” “It gives us more information about what kind of ovarian tumor Sophie may have.” “Cancer?” She looked directly at me. “It makes a malignant germ cell tumor more likely.” The hallway disappeared. All I could see was her face. All I could hear was the word malignant. Daniel whispered, “Treatment?” “If pathology confirms what we suspect, surgery may be followed by chemotherapy.” Eric covered his mouth. I looked through the small window in the recovery-room door. Sophie was asleep on the other side. Ten years old. Missing a science fair. Still wearing one glittery sock because somewhere during the emergency the other had disappeared. And suddenly I understood something. Two hours earlier, the worst thing I could imagine was that someone had hurt my daughter. Then I learned nobody had. For several minutes, I had believed that meant we had escaped the nightmare. But nightmares sometimes change shape. Dr. Shah touched my shoulder. “She has one very important thing in her favor.” “What?” “We found it.” I looked at Sophie. “We found it because of the pregnancy test.” “Yes.” The impossible positive test. The test that had caused an accusation. The accusation that had almost destroyed my marriage. The test that had terrified my daughter. And somehow that same test might have saved her life. I sat beside Sophie when she woke shortly after eight. Her eyelids fluttered. “Mom?” “I’m here.” She looked around. “Did they fix me?” My throat tightened. “They took out the thing that was hurting you.” “Good.” She closed her eyes. Then opened them again. “Daniel?” “Right outside.” “Dad?” “Also outside.” She frowned. “Are they fighting?” “No.” “Good.” She drifted back toward sleep. Then whispered, “I told you there wasn’t a baby.” A tear slipped down my cheek. “Yes, you did.” “I told the doctor too.” “I know.” “She believed me.” “Yes.” That mattered to Sophie. More than I realized. Someone had believed her before the evidence explained her. I stroked her hair until she fell asleep again. At 9:17 p.m., my phone rang. Unknown number. I nearly ignored it. Something made me answer. “Hello?” “Mrs. Morgan?” “Yes.” “This is Dr. Calder from pathology.” I stood. Daniel noticed immediately. “What happened?” I held up one finger. Dr. Calder continued. “I’m sorry to call this late, but Dr. Shah requested immediate notification if we identified anything significant.” My heart started pounding. “You have results already?” “Preliminary results.” “And?” “The ovarian tumor has characteristics consistent with a malignant germ cell tumor.” I closed my eyes. The word I had been dreading had arrived. Cancer. Before I could speak, Dr. Calder continued. “But that isn’t why I called.” My eyes opened. “What?” “It’s the second tissue sample.” “The growth near her appendix?” “Yes.” “What about it?” There was a pause. A long one. Then the pathologist said something I never expected. “The two growths are not the same.” Dr. Shah had warned us that was possible. “What is the second one?” “We’re running additional testing.” “Why?” “Because under the microscope, the tissue doesn’t resemble an ordinary pediatric tumor.” Cold spread through my body. “What does it resemble?” Another pause. “Mrs. Morgan, before I answer that, I need to ask you something about Sophie’s medical history.” “What?” “Has she ever had surgery before?” “No.” “Any organ transplant?” “No.” “Any major abdominal injury?” “No.” Daniel had moved closer now. He could hear the panic in my voice. Dr. Calder lowered his voice. “Then we have a problem.” “What kind of problem?” “The cells in the second growth do not appear genetically identical to Sophie.” For several seconds, I forgot how to breathe. “What did you just say?” “We need confirmatory testing. I want to be very clear about that.” “What do you mean they aren’t genetically identical to her?” “I mean the preliminary analysis suggests that tissue may contain DNA belonging to someone else.” The room went completely silent. I looked through the glass at my sleeping daughter. “That’s impossible.” “I agree that it is extremely unusual.” “How could somebody else’s DNA be inside my daughter?” “That is exactly what we are trying to determine.” I pressed the phone harder against my ear. “When will you know?” “We’re repeating everything tonight.” The call ended two minutes later. I remained standing beside the window. Daniel touched my arm. “What did he say?” I turned toward him. Eric was standing behind him. Both men were waiting. I opened my mouth. Then stopped. Because a memory had surfaced. Something I hadn’t thought about in ten years. Something from the night Sophie was born. A nurse entering my room at 2:13 in the morning. A sudden emergency alarm down the hallway. Sophie being taken to the nursery for less than twenty minutes. And when they brought her back, the identification bracelet around her ankle had been replaced. At the time, the nurse told me the original had broken. I had believed her. Why wouldn’t I? But now, staring at my daughter through the recovery-room glass, I remembered one other detail. A detail so small I had forgotten it completely. The nurse who returned Sophie to me that night had looked terrified. And before leaving the room, she had leaned close to my bed and whispered six words I had never understood. “Please don’t ask where she went.”….
